• Building your capacity: We'll work alongside you to develop your skills in managing your own support, and making informed decisions about your care.

• Advocating for your needs: We can help you set up service agreements and bookings. Any issues you might encounter with your providers, we're here to ensure your supports are established smoothly and delivered constantly. 

• Achieving your goals: We help you track your progress towards your goals, identify what went well, and adapt to your plan as your needs progress.

• Crisis planning and resolution: In times of unexpected situations, we can help you develop a contingency plan and connect you with the appropriate services to ensure the continuity of your care.

• Community-driven integration: We actively assist you with exploring activities within your community to help you engage with local groups and activities, fostering a sense of belonging.
